Below are the attached forms to apply for continuing education approval, both for state, national, association, and government organizations, as well as a separate form for proprietary and commercial vendors.

Pricing and Other Information

NVRA will charge a fee of $175/state and $200 for national association or government for evaluating each program. The form and all attachments must be submitted not less than 60 days prior to the event. Requests for Approval received fewer than 60 days prior to the date of the event will be assessed a late fee of $5 per day. Submission date is determined by postmark.

For proprietary and commercial vendors, there will be a $250 fee for evaluating each program. The agenda/course must be submitted for evaluation with payment not fewer than 60 days prior to the event in order to allow sufficient time for review and approval. Programs received less than 60 days prior to the date of the event will be assessed a late fee of $5 per day. Submission date is determined by postmark. Use a separate form for each event or course.
